Welcome to the passport services of the Embassy of India, Baku.

Applications for Passport and consular services can be made at the Consular Section of the Embassy, which is open from 09:30 hours to 12:30 hours on Monday-Friday except Holidays (see Homepage) . Passports, after completion of the service requested, can be collected from 15:30 and 17:30 hours on the date indicated. Fees for passport services are accepted at the Embassy in cash in AZN only [Passport fees Schedule ]. Application forms and suggested formats for affidavits etc. can be downloaded from this website or obtained from the Consular counter at the Embassy. Your passport is a valuable document. It should always be either in your custody or in the custody of a person duly authorised by you. It must not be altered or mutilated in any way.

If you have acquired foreign citizenship, please surrender the Indian passport to the nearest Indian Mission.

In accordance with International Civil Aviation Organization (ICAO) guidelines, Government of India has introduced Machine Readable Passports (MRP) in all passports office in India and Embassies abroad. The Embassy of India, Baku is also participating in a centralized project operated by Ministry of External Affairs (CPV division) for issuing machine readable passports with effect from August 26, 2007. Passports are now being printed in India and couriered to the Embassy. It would require around fifteen working days* after due processing of application to issue the passport by the Embassy to the applicant. The passport seeking applicants are hereby advised to check their passport's status and apply for any passport services much in advance.

ONLY IN EMERGENCY SITUATIONS A HANDWRITTEN PASSPORT FOR LIMITED VALIDITY, OF SIX MONTHS ONLY, WILL BE ISSUED TO ENABLE APPLICANT TO COVER URGENT TRAVEL AND THIS HANDWRITTEN PASSPORT CANNOT BE FURTHER EXTENDED. THE APPLICANT WILL HAVE TO APPLY AFRESH FOR THE NEW FULL VALIDITY PASSPORT.

[NOTE: all the passport applicants will have the facility to register and apply online at the WEBSITE (https://passport.gov.in/). THE ONLINE REGISTRATION IS NOT YET MADE AVAILABLE ON THE said WEBSITE. HOWEVER TILL THIS OPTION IS MADE AVAILABLE ON THE WEBSITE BY THE MINISTRY, THE APPLICANTS CAN APPLY WITH MANUALY FILLED UP FORMS]

New Passport on Final Expiry

Application form (Form 1) duly filled and signed to be submitted along with 4 photographs and the old passport along with a photocopy of the first and last page of the passport. Photographs should be of size 4 cm X 4 Cm and signatures should be inside the box given for the purpose in form. For more information about the photograph details please go through Photo-Specf 1 , 2

Applications for new passport can be submitted within one year before the final expiry of the current passport or if the only 2 pages are left in the applicant's passport. Applicant has the option to apply for an ordinary passport booklet of 36 pages or a Jumbo passport booklet of 60 pages

Time Taken: *

• Normally fifteen days if the Original Passport (10 year validity) issuing authority is this Embassy
• If the passport was issued by any other authority, at least 40 working days or earlier if clearance from concerned authority is received.

Duplicate Passport in lieu of lost/damaged passport

If the passport is lost, the matter should immediately be reported to the nearest local police authorities and a proper police report regarding the loss of passport should be obtained and thereafter Embassy should be approached. If you are on a short visit to Baku and have lost or damaged your original passport you can approach the Consular Section for either a Short validity passport or an emergency certificate.

Application form (Form 3) duly filled and signed to be submitted along with photograph.

• Supported by police report regarding loss of passport
• Sworn Affidavit for duplicate passport.
• Photocopy of the lost passport (if available). The application should be supported by any other document issued to the applicant in India - driving License, ration card, voters Identity Card, etc.

Time taken:

• Normally fifteen working days if the Original Passport (10 year validity) issuing authority is this Embassy.
• If the passport was issued by any other authority, at least 90 working days or earlier if clearance from concerned authorities is received.

Emergency Certificate
Form for issue of Emergency certificate to be duly filled, signed and submitted along with 4 photographs.

• Details of the flight and transiting country should be mentioned.
• Photocopy of the passport held.
• Any other document to establish Identity.

Time taken: Processed only on establishment of Identity.

Passports for Minors (< 15 years for a passport of 5 year validity)
Original and photocopy of the Birth Certificate issued by the hospital/commune parents' passports in original and copies, application form (Form 1), duly filled in and signed, four recent passport size photographs, marriage certificate(if the name of the spouse is not endorsed on the respective passport).

Please ensure that column No.28 on page No.6 of the application form (declaration of parents/guardian if applicant is minor) is duly filled in and signed by both the parents.

Passports for Children of Indian origin born in Azerbaijan

Children's name cannot be entered on their parent's passport any more. For a child to be given an Indian passport his/her birth must have been registered with the Consular Section of the Embassy within three months of birth. To apply for issue of a new passport to a child you must fill out Application form (Form 1) duly filled and signed to be submitted by either of the parents along with:

4 photographs of infants with eyes open

Photocopy of the passports of the parents.

Photocopy of the child's birth certificate issued by hospital (along with original)

Change of Name
Application form (Form 1) duly filled and signed along with 4 photographs to be submitted along with:

• Passport.
• Clippings of two leading newspaper (one in India and the other in Azerbaijan) carrying advertisement notifying the change of name and asking for any objection thereof to be reported to Embassy of India within 30 days of the publication.
• Sworn Affidavit in the prescribed format

Fee: As applicable for new passport
Time taken: 7 working Days.

Change of maiden name upon marriage or re-marriage or divorce or death of spouse
Application form (Form 1) duly filled and signed along with 4 photographs to be submitted along with:

• Passport.
• Marriage certificate (or divorce deed or death certificate) duly attested by the Home Department of the State Government concerned and counter attested by the Ministry of External Affairs, CPV Division, Patiala House Annexe, Tilak marg, New Delhi.

Affidavit in the prescribe format joint photograph (in case of marriage or re-marriage)
Fee: As applicable for new passport
Time taken: 7 working Days.

Change of Signature
Application form (Form 1) duly filled and signed along with 4 photograph to be submitted along with:

• Passport.
• Sworn affidavit (General).

Fee: As applicable for new passport
Time taken: Normally 3 working days

Change of any particular mentioned under lamination cover - Correction of spellings of names, Date of Birth etc.
Application form (Form 1 or Form 2) duly filled and signed along with 4 photographs to be submitted along with:

• Passport
• Documentary proof to affect the change attested by the Home Department of the State Government concerned and counter attested by the Ministry of External affairs, CPV Division, Patiala House Annexe, Tilak Marg, New Delhi.

Fee: AZN 40 if new booklet is required to be released or AZN 10 if an endorsement on the existing passport would suffice
Time taken: Normally 30-45 working days

Deletion of spouse's name

Inclusion of spouse's name

Emmigration Clearance Not Required

Diploma Certificate or proof of having stayed abroad for more than three years or proof of having paid Income Tax in India for the last three years. The Degree/Diploma Certificate should be duly attested by the designated Department of the State government concerned and counter attested by Ministry of External Affairs, CPV Division, Patial House Annexe,Tilak Marg, New Delhi

Extension of validity of passport
Application form (Form 2) duly filled and signed along with 4 photographs to be submitted along with:

Time taken: same day if passport issued by our Embassy else till the clearance is received from the original passport issuing authority
